Senior Manager, Error Reporting Programs

Michael J. Gaunt, PharmD is a Senior Director of Error Reporting Programs and Editor at the Institute for Safe Medication Practices (ISMP), which he joined in 2004. Michael supervises the ISMP National Medication Errors Reporting Program (ISMP MERP) and the ISMP National Vaccine Errors Reporting Program (ISMP VERP). In this role, he works collaboratively with the US Food and Drug Administration (FDA) to advocate for nomenclature, packaging, and labeling improvements to reduce the risk of error. He participates in on-site risk assessments in health care systems, particularly their ambulatory care areas, and community pharmacies to assess risks and identify opportunities to improve safety and reliability. Michael also serves as the editor of the monthly ISMP Medication Safety Alert!® Community/Ambulatory Care newsletter.

Michael represents ISMP at several organizations and partnerships, including the National Coordinating Council for Medication Error Reporting and Prevention (NCC MERP), the Centers for Disease Control and Prevention (CDC) PROTECT Initiative, and the Partnership for Health IT Patient Safety. Michael has made contributions to medication and health information technology safety science as a member of various grant-funded research teams. He has published articles in the patient safety and pharmacy literature, including a monthly column in Pharmacy Times.

Michael received a Bachelor of Arts in History from the University of Notre Dame and a Doctor of Pharmacy with a Certificate in Drug Safety from Temple University School of Pharmacy.
