Medication Safety Officer, Concord Hospital

Elizabeth Wade, PharmD, BCPS is the Medication Safety Officer at Concord Hospital, New Hampshire. Liz has been a leader in advocacy, education, and research in medication use safety at local, state, and national levels.  She has served on ASHP’s Section of Inpatient Care Practitioners Section Advisory Group on Medication Safety, as a New Hampshire representative on the ASHP House of Delegates, and on the ASHP Council on Pharmacy Practice.  She is a past president and active member of the board of directors for the New Hampshire Society of Health-System Pharmacists.  She works closely with ECRI, a founding member of the Partnership for Health IT and Patient Safety, in development of national best practice guidelines in medication safety and informatics.  She worked actively with the New Hampshire Board of Pharmacy to pass legislation for implementation of continuous quality improvement programs in all pharmacies in New Hampshire.

Her publications include, “The frequency of intravenous medication administration errors related to smart infusion pumps: a multihospital observational study,” in British Medical Journal Quality and Safety, “Discrepancies between structured and free-text ordering fields in computerized provider order entry systems,” in the Journal of the American Medical Informatics Association, which received the Distinguished Paper Award from the American Medical Informatics Association, and a book chapter in The Joint Commission’s Value of Close Calls in Improving Patient Safety on medication displacement in automated medication dispensing cabinets.  Her recognitions include Distinguished Young Pharmacist of the Year in New Hampshire in 2014, the Maryland Society of Health System Pharmacists Medication Safety Award, and the ASHP Student Leadership Award in 2007.

Liz earned her Doctor of Pharmacy degree from Northeastern University. She completed both a residency in Pharmacy Practice, and a Medication Use Safety Residency at The Johns Hopkins Hospital.
