Enhancing Medication Safety: The Role of Safe Labeling,
Bar Coding and Outsourcing of IV Products

PROGRAM OVERVIEW Many medications and unique solutions administered via continuous IV infusion or epidural infusion are not commercially available, requiring hospital staff to prepare, label, and dispense those products. Some hospitals and pharmacy departments are experiencing difficulty meeting the new regulations by the United States Pharmacopeia Chapter 797 and The Joint Commission governing onsite preparation of medications and IV admixture solutions. This teleconference will present the requirements for clear, accurate, and user-friendly product labeling to reduce the potential for preventable adverse medication events with IV drug products and infusions. Label guidelines for IV drugs and infusions developed by ISMP will be presented. In addition, the application of barcode technology as a high-leverage error reduction strategy in the preparation, tracking, final product labeling, dispensing, and administration of IV drug products will be reviewed. Participants will be informed about why, when, and how to outsource IV products as a safety strategy by a practitioner experienced with outsourcing.

Registration

• One telephone line connection is allowed per registration. Others may listen on this telephone line via an internal extension, or a speakerphone may be used for larger groups.
• You may not set up conferencing capabilities for other sites outside the facility. Each specific site may be used for larger groups. Each specific site must register individually.
• We cannot accept registrations without an email address and the name of a contact person.
• Registration for this teleconference will end at midnight ET on 3/11/2009.
• CDs will be available for purchase after the teleconference.

Cancellations

• Cancellations must be requested via an email message to our teleconference department.
• Cancellations received before the start (1:30 pm ET) of the teleconference will receive a full refund or credit.
• Cancellations received after the start (1:30 pm ET) of the teleconference and within 3 business days after the teleconference will be assessed a cancellation fee of $50.00.
• If your organization does not cancel within 3 business days after the date of the teleconference and does not participate, the full registration fee will be charged.

• If your organization's registered participant does not receive the teleconference materials that are sent out approximately 2 days before the teleconference via email, please send an email message to our billing department prior to the start of the teleconference.
