Turning a New Chapter on IV Drug Compounding Safety

PROGRAM OVERVIEW

The United States Pharmacopeia (USP) has published the first enforceable national standards for sterile compounding - USP Chapter 797. Based on this, many boards of pharmacy will be adopting new regulations, the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) will be surveying for compliance, and the FDA is considering new legislation. Two renowned speakers held a teleconference on July 15, 2004 on the new national standard for sterile preparation.. The first speaker, Lawrence A. Trissel, from M.D. Anderson Cancer Center in TX, provided an overview of the USP Chapter 797, define the responsibilities of compounding personnel, and described how to interpret the quality assurance components that need to be considered and addressed to protect patients from erroneous and contaminated sterile compounding preparations. The next speaker, Darryl S. Rich, PharmD, MBA, from the Joint Commission, offered the JCAHO's position on the USP Chapter 797, and described how the JCAHO will survey for compliance with the requirements in 2004. A 30-minute question and answer period will followed the 60-minute presentation. Continuing education credit are provided for nurses and pharmacists.

OBJECTIVES

Following completion of this teleconference, participants should be able to:

Demonstrate an understanding of the implications of the USP Chapter <797> national standards for quality assurance of compounded sterile preparations in pharmacies and other locations.
Describe the environmental engineering controls and monitoring requirements to assure an adequate environment for the compounding of sterile preparations.
Describe the various risk levels indicated in USP Chapter <797> for differing compounded sterile preparations and the quality assurance requirements of each.
Explain the processing and quality assurance requirements of compounding sterile preparations from nonsterile raw ingredients, and how that differs from aseptic processing.
Describe the regulatory authority of USP-NF Chapters 0-999, with regard to both federal and state laws and regulations.
Describe the position of the JCAHO and how it will survey for compliance with regard to USP-NF Chapter 797.

Registration

One telephone line connection is allowed per registration. Others may listen on this telephone line via an internal extension, or a speakerphone may be used for larger groups.
You may not set up conferencing capabilities for other sites outside the facility. Each specific site may be used for larger groups. Each specific site must register individually.

Cancellations

Cancellations must be requested via an email message to our teleconference department.
Cancellations received before the start (1:30 pm ET) of the teleconference will receive a full refund or credit.
Cancellations received after the start (1:30 pm ET) of the teleconference and within 3 business days after the teleconference will be assessed a cancellation fee of $50.00.
If your organization does not cancel within 3 business days after the date of the teleconference and does not participate, the full registration fee will be charged.

Materials

If your organization's registered participant does not receive the teleconference materials that are sent out approximately 2 days before the teleconference via email, please send an email message to our billing department prior to the start of the teleconference.
When registering, please be sure to provide the email address of the person to whom ISMP should send these materials.
