ISMP Survey on High-Alert Medications in Community/Ambulatory Healthcare

ISMP is updating the ISMP List of High-alert Medications for Community/Ambulatory Healthcare. Please take a few minutes to complete the short survey on high-alert medications (see definition below) and submit your responses by October 31, 2017. High-Alert Medications: Medications that bear a heightened risk of causing significant patient harm when they are used in error. Although errors may or may not be more common with these medications, their direct consequences are often more devastating.
