This tool was developed to assist community pharmacies in analyzing their current status with implementing the ISMP Targeted Medication Safety Best Practices for Community Pharmacy. For each of the best practices, this worksheet allows pharmacies to document their implementation status and detail their plan for full implementation. ISMP encourages pharmacies to focus their medication safety efforts on these best practice guidelines, and this worksheet may help identify gaps in safety practices and develop an action plan to address any vulnerabilities.
