The Fellowship application is available in:

• a Microsoft Word formatted document that can be saved, printed and manually filled out, and
• a PDF form that can be filled out on your computer. In order to use the fillable features, please note the following:

Requirements for downloading forms:

You MUST have a copy of Adobe Acrobat Reader 4.0 (or later) installed on your computer in order to view and print these forms. It is also necessary to use either Netscape 6.0 or Internet Explorer 5.0 or later versions in order to accurately view or download these forms. Earlier versions of these browsers, as well as others, do not have the "font catching" capabilities needed to properly use these forms.

Steps to download and save form for data entry:

1. Update to Current version of Adobe Acrobat or Adobe Reader

To update Adobe Reader:
a. Download the update from the Adobe Web site at www.adobe.com/products/acrobat/readstep2.html .

To update Acrobat:
a. Start Acrobat
b. Choose Help > Update.
c. Follow the on-screen instructions to install the update.

Download the update from the Adobe Web site at www.adobe.com/support/downloads/main.html.

2. Complete the form, print, sign, and mail.

You may only save a blank form on your local hard drive. In order to complete the form, gather all necessary information and enter it in one sitting. Then, print the form and sign the last page before mailing.
