ISMP has published numerous reports of harmful and even fatal errors that have occurred as a result of improper preparation of sterile intravenous (IV) products. Error reports involving IV compounding have also appeared in mainstream consumer media outlets, thus raising safety concerns with IV compounding to the level of a serious public health issue. This symposium will provide education for pharmacists on the use of new technology to prevent errors with IV admixtures.

Speakers will present background on some of these errors and how they may have occurred as well as recommendations to prevent these errors including improving pharmacy practices and staff training programs as well as the use of bar coding and digital imaging in IV admixture preparation to guide, document, and check each step of preparation remotely. Users of the three major vendors offering this new technology will discuss their experiences in implementing software and how they have optimized the safety benefits.
