Program Overview

Many medications and unique solutions administered via continuous IV infusion or epidural infusion are not commercially available, requiring hospital staff to prepare, label, and dispense those products. Some hospitals are experiencing difficulty meeting the new regulations governing on-site preparation of medications and solutions via continuous IV or epidural infusion, such as the United States Pharmacopeia Chapter 797 and the Joint Commission standards for IV admixtures to be prepared by the pharmacy department.

ISMP’s symposium will present the requirements for clear, accurate, and user-friendly product labeling to reduce the potential for preventable adverse medication events with IV drug products and infusions. Guidelines developed by ISMP for the design of labels applied to IV drugs and infusions will also be presented.

In addition, the application of bar-code technology as a high-leverage error prevention strategy in the preparation, tracking, final product labeling, dispensing, and administration of IV drug products will be reviewed.  Attendees will be informed about why, when, and how to use outsourcing as a safety strategy by a practitioner familiar with outsourcing.
