  • Oral vaccine mistakenly given by injection. (rotavirus vaccines)
    • The rotavirus vaccine is the only oral live vaccine available in the US market. Providers with less experience administering oral vaccines could inadvertently inject them, especially one formulation that needs to be reconstituted.
  • Welcome action by CVS. (not selling tobacco products)
    • ISMP acknowledges CVS Caremark Corporation for its recent announcement that the company will stop selling tobacco products in their retail pharmacies.
