  • Baxter heparin recall.
  • FDA has issued a public health advisory about severe allergic reactions involving heparin manufactured by Baxter. More information about the recall and strategies to promote safest use of heparin products is included in this SafetyBrief.

  • Scratch IV colchicine.

  • USA Today news series: Clarifying the issues and embracing community pharmacy safety.
  • Look-alike vials.
  • Look-alike vials of Zemplar and fosphenytoin from Hospira have been mixed-up in storage areas in at least one clinical setting. 

  • Definitely not a Band-aid!
