  • Safety Briefs:
    • The December 3, 2001, Federal Register includes a government proposal that requires a bar code on packages of human drug and biological products beginning some time next year. In addition to the drug name and dose, the bar code would contain certain information such as the National Drug Code number, and possibly the expiration date and lot number, to help identify expired and recalled drugs.
    • Several boxes of ENGERIX (hepatitis B vaccine) were issued to an employee health service for staff vaccinations. Unfortunately, in place of the adult formulation of 20 mcg/mL, the pediatric/adolescent dose of 10 mcg/0.5mL was accidentally issued. The word "PEDIATRIC" is missing on the pediatric strength carton but the word "ADULT" is lised on the adult strength.



    • When a pharmacist spoke with a nurse regarding an order for BOOST BAR, a popular nutritional energy bar from Mead Johnson Nutritionals, the nurse thought he said "BUSPAR" (buspirone).
    • Survey on Pharmaceutical Bar Coding and Unit Dose Packaging in Hospitals
