ISMP® Medication Safety
Self Assessment®
for Community/Ambulatory PharmacyTM
Posted are the preliminary aggregate data from the ISMP®
Medication Safety Self Assessment® for Community/Ambulatory
Pharmacy. If you have completed the self-assessment and transmitted
your results to ISMP, you will be able to view the data by
proceeding to the "Results" link below and entering
your unique password from your completed assessment. The password
is contained on the first page of your printed scores. The
results are presented by type of pharmacy (independent, chain
or other) and by prescription volume (less than 700 prescriptions
per week, between 701 and 1500 per week or greater than 1501
per week). Other demographic comparisons will be provided
in the future.
When viewing the aggregate scores use your computer-generated
survey results, which were created when you submitted data
to ISMP. Compare your scores for each of the ten key elements,
20 core characteristics, and total score to the results that
match your type of pharmacy and/or prescription volume.
We have also provided a comparison of the mean percentage
of the maximum weighted score for each key element, core characteristic,
and total. For example the maximum weighted score for key
element ONE was 80 and the mean aggregate score for all participants
was 38. This corresponds to a mean percent of the maximum
weighted score of 48%. You can compare your scores to the
mean percent of the maximum weighted score by dividing your
total score by the maximum total score listed at the top of
each key element, core characteristic, or total.
We encourage you to share the results with the team you assembled
to complete the self-assessment and your entire staff. Use
the data to compare your organization to other demographically
similar pharmacies. However, please do not only rely upon
your results compared to others to decide whether you need
to improve medication safety in certain areas. All scores
are relative and cannot be used to predict which pharmacies
are safe. Thus, if your performance is better than others,
do not be lulled into complacency. Instead, use the comparative
data to stimulate your ongoing efforts to fully implement
all the medication error reduction strategies suggested in
the self-assessment.
While it is important to widely disseminate and use the preliminary
data from the ISMP® Medication Safety Self Assessment®
within your organization, please refrain from publishing or
distributing the data externally. Unauthorized release of
the data, which is protected by copyright, may result in misinterpretation
of the data.
Again, we thank you for participating in the ISMP® Medication
Safety Self Assessment® and commend you for submitting
your findings to us. We are well aware of the challenges you
faced in both completing the assessment and sharing your findings.
The ultimate goal of the ISMP® Medication Safety Self
Assessment®has been to heighten awareness of distinguishing
characteristics of a safe medication system and to create
a baseline for medication safety efforts that can be evaluated
over time. Additionally, ISMP, the National Association of
Chain drug Stores (NACDS), the American Pharmaceutical Association
(APhA) and other national organizations plan to further analyze
the aggregate data to develop curricula to support your ongoing
efforts to enhance medication safety. Without your help, we
would not be able to achieve these goals. In the end, we firmly
believe that your collective willingness to share your assessment
of medication safety will ultimately enhance the public's
confidence as we work together to make our healthcare systems
safer and more efficient.
