ISMP® Medication Safety
Self Assessment®
for Community/Ambulatory Pharmacy
Posted are the preliminary aggregate data from the ISMP®
Medication Safety Self Assessment® for Community/Ambulatory
Pharmacy. These results contain data from those sites that
submitted their assessment results prior to July 2005. The
results are presented by type of pharmacy (independent, chain
or other) and by prescription volume (less than 700 prescriptions
per week, between 701 and 1500 per week or greater than 1501
per week).
When viewing the aggregate scores use your computer-generated
survey results, which were created when you submitted data.
Compare your scores for each of the ten key elements, 20 core
characteristics, and total score to the results that match
your type of pharmacy and/or prescription volume.
We have also provided a comparison of the mean percentage
of the maximum weighted score for each key element, core characteristic,
and total. For example the maximum weighted score for key
element ONE was 80 and the mean aggregate score for all participants
was 38. This corresponds to a mean percent of the maximum
weighted score of 48%. You can compare your scores to the
mean percent of the maximum weighted score by dividing your
total score by the maximum total score listed at the top of
each key element, core characteristic, or total.
We encourage you to share the results with the team you assembled
to complete the self-assessment and your entire staff. Use
the data to compare your organization to other demographically
similar pharmacies. However, please do not only rely upon
your results compared to others to decide whether you need
to improve medication safety in certain areas. All scores
are relative and cannot be used to predict which pharmacies
are safe. Thus, if your performance is better than others,
do not be lulled into complacency. Instead, use the comparative
data to stimulate your ongoing efforts to fully implement
all the medication error reduction strategies suggested in
the self-assessment.
While it is important to widely disseminate and use the preliminary
data from the ISMP® Medication Safety Self Assessment®
within your organization, please refrain from publishing or
distributing the data externally. Unauthorized release of
the data, which is protected by copyright, may result in misinterpretation
of the data.
We thank you for participating in the ISMP® Medication
Safety Self Assessment®. We are well aware of the challenges
you faced in completing the assessment. The ultimate goal
of the ISMP® Medication Safety Self Assessment®has
been to heighten awareness of distinguishing characteristics
of a safe medication system and to create a baseline for medication
safety efforts that can be evaluated over time. Additionally,
ISMP, the National Association of Chain drug Stores (NACDS),
the American Pharmaceutical Association (APhA) and other national
organizations plan to further analyze the aggregate data to
develop curricula to support your ongoing efforts to enhance
medication safety.
