ISMP®
Medication Safety Self Assessment® for Hospitals
The 2004 edition of the ISMP® Medication Safety Self
Assessment® for Hospitals is now
available. This cooperative
effort by the Institute for Safe Medication Practices, the
American Hospital Association, and the Health Research and
Education Trust is made possible through a grant from the
Commonwealth Fund.

The report includes an aggregate profile of hospital respondents,
a list of the maximum weighted scores possible for each representative
self-assessment characteristic, and aggregate comparative
reports on the key elements and core distinguishing characteristics
of safe medication practices. Findings were also published
in the November issue of Joint Commission Journal on Quality
and Safety (Smetzer JL, Vaida AJ, Cohen MR, et al.: Findings
