  • Pump up the Volume – Tips for increasing error reporting.  Error reporting is a fundamental component of a safety culture, but persuading healthcare workers to submit reports is no easy task due to the perceived barriers to reporting. This issue discusses the reasons for poor reporting and offers some key elements of an reporting program that encourages error reporting.
  • New CDC Website for Long-term Care Facilities. Discusses a new Centers for Disease Control and Prevention (CDC) website with infection prevention resources for long-term care facilities.
  • Worth Reading – Suggested publication related to medication safety.  Cites a recent article on zinc supplementation in order adults with wounds. 
  • SafetyWires: Discusses reported errors related misinterpretation of dose caused by lines on orders sheets; inadvertent use of an EpiPen training device instead of active drug; and holding Lantus insulin orders. Suggested strategies to prevent similar errors are discussed.
