  • Computer medication order entry: Survey analysis (Part 1). This article discusses the results of a recent survey of the ISMP Long Term Care Advise-ERR readers related to computer medication order entry systems.  Results are discussed showing that technology uptake is increasing but maximization of safety features are needed.
  • SafetyWires: Discusses three medication safety issues in long-term care facilities caused by the use of the symbols for greater than (>) and less than (<); placing the dosing guidelines in the Order Comment field of the computer order entry system so it does appear on the MAR; and potential errors cause from poor labeling of the FIRST brand of oral vancomycin.
