The following are excerpts from the newsletter

April 24, 2014

In this week's issue:

  • Part II: Disrespectful behaviors Their impact, why they arise and persist, and how to address them
  • AUVI-Q post injection ticking sound.
    • We were recently alerted that AUVI-Q auto-injector’s (EPINEPHrine injection) electronic voice speaker may make a “ticking” sound as the battery drains and can’t fully power the device. This sound might alarm someone who thinks the ticking noise is associated with a possible calamitous event.
  • Solid controls needed for demo training devices.
    • In addition to the accidental storage or demo products in patient care areas, we recently learned about a different issue with demo devices that is a major cause for concern. Dozens of students from a university injected each other with solution from vials of a Pocket Nurse Demo Dose product meant to resemble 0.9% sodium chloride injection.
  • Certification for medication safety officers?
    • ISMP is collaborating with the American Society of Health-System Pharmacists (ASHP) and the National Patient Safety Foundation (NPSF) to determine the need for a certification credential for medication safety officers/leaders. Please complete a short 3-5 minute survey by May 5 at:  www.ismp.org/sc?id=348.
