  • Take a tour of ISMP's website: Provide your patients with a link to the site
  • New connectors coming for enteral feeding tubes
    • Later this year, the International Organization for Standardization (ISO) will introduce new design standards for enteral feeding device connectors.
  • The mL-only standard for liquid dosing gathers steam.
    • The National Council for Prescription Drug Programs (NCPDP) just released a white paper supporting milliliter (mL) as the standard unit of liquid measure on prescription container labels for oral liquid medications. It also calls for dosing devices with numeric graduations and for units that correspond to product labeling. 
  • MARQIBO and risk of errors.
    • Fatal mix-ups have occurred between liposomal and conventional formulations of various drugs. Although we haven’t learned of any actual events involving vinCRIStine sulfate liposome injection (MARQIBO) and conventional vinCRIStine sulfate injection, a mix-up could happen.
  • ANGELIQ-not a birth control pill!
    • Angeliq is a hormone-based medicine, used to treat symptoms of menopause. Unfortunately, we’re aware of errors where it was prescribed improperly as an oral contraceptive.
