Independent Double Checks: Undervalued and Misused


NAN Alert

ISMP and the American Society of Health-System Pharmacists (ASHP), in cooperation with the National Coordinating Council for Medication Error Reporting and Prevention (NCC MERP), distributed an alert earlier this week about an important labeling change for heparin products. The alert ( was sent via the National Alert Network (NAN).

Another heparin label issue?

Unfortunately, with the new heparin label as described in the NAN alert, another issue has come to light.

“Patient friendly” service or danger up ahead?

Dispensing samples and starter doses are often seen as “patient friendly” services, but the services can also have unintended consequences.

Don’t forget to filter Cathflo Activase

CATHFLO ACTIVASE (alteplase) is available again, but the US Food and Drug Administration (FDA) asked us to remind healthcare practitioners that the final reconstituted solution must be filtered with a 5-micron filter needle upon withdrawal from the container
