  • QuarterWatch™2011 Annual Report
    An estimated 2 to 4 million drug-induced serious injuries in 2011
  • Safety Brief:  Drainage system has Luer connector. The ASEPT drainage system for pleural effusions and malignant ascites (B. Braun Interventional Systems) provides a convenient method for handling chest and abdominal drainage in the hospital or at home. The drainage catheter incorporates a swabable self-sealing valve to prevent leaks.

  • Safety Brief: CMS citing reuse of insulin pens. On May 18, 2012, the Centers for Medicare & Medicaid Services (CMS) became the latest federal agency to address the reuse of insulin pens. The agency recently announced that hospitals will be issued a citation when surveyors identify incidents of insulin pen sharing. 

  • Safety Brief: Veterinary medication error reporting.  ISMP occasionally receives medication error reports that involve pets and even livestock. Many of these errors are similar to what would be seen with medication errors that affect people and, thus, may have implications for human medicine.

  • ISMP webinar.  Join us on June 14 for The Joint Commission Medication Management Update for 2012.Our speaker, Darryl Rich, PharmD, MBA, will be reviewing The Joint Commission’s 2012 requirements associated with medication management standards and medication-related national patient safety goals. 

  • ISMP webinar.  We will be presenting Patient Information Matters: Reducing Harmful Errors Associated with Weights and Allergies. Communication breakdowns described in adverse drug event reports analyzed by ISMP frequently involve failures to obtain, document, and update accurate patient information—most commonly allergies and/or weight measurements.
