Leucovorin Dosing Error in Lexi-Comp’s Drug Information Handbook (13 th edition) and Drug Information Handbook for Oncology (5th edition)
Lexi-Comp’s Drug Information Handbook, 13th edition (pg 872) and Drug Information Handbook for Oncology, 5th edition (pg 498) contain a dosing error in the leucovorin monograph. The Rescue Dose section of the Dosage field incorrectly labels the units for serum methotrexate concentrations (used as a guide for dosing the leucovorin) as “ m mol/mL” instead of “ m mol/L”.
The revised text reads as follows ( changes in bold):
Rescue dose: Initial: I.V.: 10 mg/m 2 , then:
Oral, I.M., I.V., Subcutaneous: 10-15 mg/m 2 every 6 hours until methotrexate level is less than 0.05 micromole/L; if methotrexate level remains greater than 5 micromole/L at 48-72 hours after the end of the methotrexate infusion, increase to 20-100 mg/m 2 every 6 hours until methotrexate level is less than 0.05 micromole/L
These changes have been automatically posted to their online and hand-held reference sources, but the error will persist in the current print editions. Some older print editions also contain this recently identified error (including Drug Information Handbook, 12 th edition; Drug Information Handbook for Oncology, 3rd and 4th editions).
