Canadian Publication Errata: Compendium of Pharmaceuticals and Specialties (CPS) 2008

Corrections in CPS 2008-English:

• Erratum: Clin-Info section, Calculations and Dosing Tools, p. L3: There is an error in the first equation for body surface area (BSA) at the top of the page that could result in underdosage. The square brackets should enclose both the numerator and denominator in the equation for BSA. Please print the corrected pdf of the page and insert it in your copy of CPS 2008. The correct equation is as follows:
  
  
• Erratum: Opioids Monograph: On page 1591, the entry for Hydrocodone in the Summary Product Information table should be Hydromorphone. Hydrocodone is available as single ingredient 5 mg tablets and 5 mg/mL syrup as well as a component of multi-ingredient products.
• Erratum: OxyContin: On page 1628, under Supplied: OxyContin: 5 mg, the words controlled release should be added.
• Erratum: Sulfamethoxazole-Trimethoprim CPhA Monograph: On page 2167, the therapeutic class should read: Antibacterial.

Corrections in CPS 2008-French:

• Erratum : Section Info-Clin, Calculs et outils posologiques, p.L3 : Il y a une erreur dans la première équation pour calculer la surface corporelle (SC) présentée au haut de la page qui pourrait résulter en une dose insuffisante. Les crochets devraient entourer à la fois le numérateur et le dénominateur dans l'équation pour la SC. Prière d'imprimer la version pdf corrigée de cette page et de l'insérer dans votre exemplaire du CPS 2008. La bonne équation est la suivante :
  
  
• Erratum : Monographie des Opioïdes : A la page 1792, l'entrée pour l'Hydrocodone dans le tableau Renseignements sommaires sur le produit devrait lire Hydromorphone. L' hydrocodone est disponible en tant qu'ingrédient unique en comprimés de 5 mg et en sirop (5 mg/mL) ainsi qu'en produits combinés.
