Textbook and Publication Errata

Mosby's Nursing Drug Reference 2005 contains inaccurate information regarding phenytoin administration.

On page 825 of Mosby's Nursing Drug Reference 2005, the following directions for administration of phenytoin are provided:

"After diluting with diluent provided 2.2 ml/100 mg, 5.2 ml/250 mg, 1 ml/50 mg); shake; place vial in warm water to dissolve powder; give through y-tube or 3 way stopck; inject slowly <50 mg/min; clear IV tubing first with NS sol; use inline filter; discard 4 hour after prparation. Inject into large veins to prevent purple glove syndrome."

This is incorrect - phenytoin does not come as a powder for dilution prior to IV administration. The publisher has been notified.
