This document provides information on submitting a CHEERS AWARD nomination electronically.
Nomination Submission Overview
The basic process to submit a nomination electronically is as follows.
Complete the Demographics Sheet 1 page. This includes the type of nomination (healthcare professional, nomination submitter's information as well as the nominee's information. This page also includes a place to type in a descriptive narrative and/or to upload a file that contains the descriptive narrative.
If you selected "Healthcare Professionals" in the Demographics form then you will have to complete the Demographic form for Healthcare Professionals.
Upload (attach) any supporting documents.
Confirm your information.
Fax or mail leadership and Interdisciplinary Commitment declarations if applicable. The fax number is 215-914-1492 and the mailing address is 200 Lakeside Drive, Suite 200, Horsham, PA 19044.
You might find it very helpful to review the nomination checklist to insure that you meet all requirements. You might also review the criteria for each nomination type to ensure that you select the proper nomination type. The nomination categories are as follows.

Here you choose whether this nomination is for an individual or small group, healthcare organization, or professional organization/group collaborative. Note that if you select "Healthcare Organization" then you will also have to specify either "Healthcare Organization initiative" or "Subscriber Award"
Simply click one of the radio buttons
to make your selection.
Note that you could also click on the title link for each nomination type to review more information about that particular type.
Note also that you must specify the nomination category or you will not move to the next step in the nomination process.
Submitter Information
Figure 2 shows a screen shot of the submitter information section.

This section is where you put your (the submitter or one doing the nominating) information. All fields are required except for the suite and fax number.
Nominee Information
Figure 3 shows the nominee information section of the form.

Use this section to provide the information for the individual, group, or organization that is being nominated for an award. Notice the checkbox (Check the box if this is a self nomination) and the button at the top of this section. The check box is used to copy the submitter information to the nominee information section. This is used for a self-nomination. Use the "Clear Nominee Information" button to clear all nominee information fields if you should click the checkbox and therefore accidentally copy the submitter information to the nominee information section.
All fields are required except for the Suite/Apt and fax number.
Descriptive Narrative
Figure four (4) shows a screen shot of the descriptive narrative section.

Uploading the descriptive narrative: Select a file on your computer's hard drive to upload to the ISMP website. Simply type the full path of the file that you want to upload or browse for the file by clicking the "Browse" button. See the uploading a file instructions for more detailed information.
Click the "Next >>" button to move to the next step of the nomination process. If a required field is missing then you will receive a message indicating the missing field. Simply provided the required information and click the "Next >>" button again.
The Demographic for Healthcare Organizations form
Figure 1 shows the top portion of this form (web page).

Simply click the radio buttons that correspond with the answer for each of the questions and click the "Next >>" button at the bottom of the form (not shown in image above).
uploading supporting files
Figure 5 shows a screen shot of a portion of the Supporting Documents upload page. Use this page to upload additional documents that pertain to this nomination.

You can upload up to five (5) additional supporting documents with this page. Simply provide a description for the file and the full path of the file, which is located on your computer and will be uploaded to the ISMP website. See instructions for uploading files for more information. Notice that at the top of the page (see figure 1) that there is a link that you can use to get more information on uploading files.
The method of uploading files here is the same as for the descriptive narrative.
Click the "Next >>" button when you have specified the supporting documents that you wish to upload.
Review Your Entries
The next page allows you to review the information that you have provided in the nomination process. Simply click the (Edit) link to change any values that you wish to modify. Figure 6 below shows a screen shot of the top portion of the review page.

Click on the "Submit Nomination" button to submit your nomination to ISMP. You will then see the "Thank you" page. Figure 7 below shows a screen shot of the thank you page.

If you selected the Healthcare Organizations category then you will also have to mail of fax the leadership and interdisciplinary declarations to ISMP. Mail them to:
